To do my work as best as possible, I create a paper file of our therapy sessions. This file contains notes about the conversations, sometimes about the content, sometimes about the form, and sometimes about my observations and interventions. This helps me to quickly see how the therapy is going and what agreements we have made.

Your file may contain information necessary for your guidance. I only request information from healthcare providers, such as your GP, after your explicit permission. Your file is accessible to you.

I handle your file with care, unauthorized persons do not have access to this data.

I can also use the information from your file:

- To inform other healthcare providers, for example when therapy has been completed or when a referral is made to another practitioner. This only happens with your explicit permission.

- Anonymized, I use the therapy process for peer review, supervision, and intervision.
- A small part of the data is used to prepare an invoice.
- To keep you informed of my activities.

If I want to use your data for another reason, I will first inform you and ask for explicit permission.

Working with a file is a legal obligation for me as a therapist and is described in the Medical Treatment Agreement Act (WGBO).

Your file will be kept for 15 years, as required by law. Perhaps it is also good to know that I have a legal obligation of confidentiality.

Privacy on the note

The invoice you receive contains your name, address, and place of residence. There is also a brief description of the therapy, e.g. Gestalt therapy, supervision, or Gestalt coaching. This invoice ultimately ends up at my administration office. I have concluded a privacy agreement with them, in which they state that they won’t take over or distribute client data.

If you have any questions about this, I would be happy to discuss it with you.
